Notice: Discontinuation of the Continuum Health eGateway

Dear Valued Patient, As part of our ongoing efforts to improve the care you receive at Continuum Medical Care, we are upgrading to a new Electronic Medical Record system. The new system will allow us to streamline a number of aspects of the care you receive at Continuum. For example, once the new EMR is fully implemented, there will be a much-improved online appointment booking system which will make it easier to book appointments with your family doctor or in our walk-in clinic. We will also be launching video visits and remote health monitoring in the near future.

The new EMR system does not have the capacity to share your medical records with you. We know that this is an important feature for some, and we are working towards a solution. In the meantime, we apologize for this inconvenience. If you currently have access to your medical information through this portal, we would encourage you to print them off so you have a personal record. Please note that the new online booking service will not be operational for approximately 6-8 weeks as we transition to the new EMR system. During this time online booking will not be available.
